In individuals who have been diagnosed with herpes encephalitis and have not responded to antiviral medication, anti-N-Methyl-D-Aspartate(NMDA) receptor encephalitis may be investigated. We were contacted by a patient who had been treated for herpes encephalitis but had developed agitation, psychosis, and dyskinesia following therapy. Anti-NMDA receptor encephalitis was suspected based on intracranial magnetic resonance imaging (MRI) and electroencephalography (EEG) results. The treatment of immunotherapy began immediately, and cancer research began with the discovery of a positive anti-NMDA receptor in the cerebrospinal fluid, but the outcome was negative. Anti-NMDA receptor encephalitis can be difficult to diagnose. In individuals with encephalitis, MRI and EEG data are critical in determining the differential diagnosis. When anti-NMDA receptor encephalitis is diagnosed, cancer investigations are negative.
